Submit all required immunization information. You cannot attend classes without meeting the state law requirements.

INSURANCE
Once registered for classes, either enroll in, or waive out of, the Student Health Insurance Plan. Visit our SHIP page for more info and to take action..
If you are waiving out, provide a copy of the front AND back of current insurance information by uploading it to myPIRATEchart.
UNDER 18 CONSENT
Students who are under 18 years of age need a treatment consent on file to receive services at SHS.
Legal parent/guardian must complete this Consent for Treatment of Patients Under 18.
